How to Enter

Application Details

Please follow the below instructions carefully when submitting your entry as incorrect applications will not be considered. Completed forms must be received in both email and hard copy no later than 5pm on 26 February 2010.

All entries received after this time may not be considered.

ALMOs may apply for more than one category, but can make a maximum of two applications per category.

Please type the answers into the award application form in Arial 10pt, line spacing 1.5. There is a maximum of three pages of A4 for the whole form and strictly 500 words in the description section. Please print 2 copies of the application form and send together with your supporting information to the below address:

Please also send one copy of each application form to nfaapplication@acceleris-mc.com with the reference number and ALMO name in the subject title.

Please quote your unique reference number on all correspondence.

Guidance notes to application form

The below information will help you when answering the questions and preparing the response the judges will be evaluating.

When you click on the category you wish to enter you will be asked for your email address and name. A copy of the application form and a unique reference number will be sent to the email address supplied (please ensure this is correct).

Reference Number

Please enter the reference number supplied in the email when you request the form.

Category

This will automatically be generated when you click on the category you wish to apply for.

1. Name and location of ALMO

Specify the full name and location of your ALMO. We will refer to your ALMO exactly as it appears in the application form in all correspondence and press, so please ensure it is accurate.

2. Details of person submitting nomination

Please provide the full name, address, telephone number, email address and (where applicable) job title of the person responsible for submitting the nomination.

3. Name of individual and, if required, job title nominated for award (individual awards only)

Please supply the first and last name of the individual nominated.

4. Project title (for project awards only)

Please supply a project title. We will refer to your project title exactly as it appears in the application form in all correspondence and press, so please ensure it is accurate.

5. Please describe the project / individual

There is a strict 500 word limit in this section. Please provide your word count used at the end of this section.

Describe the results/outcomes that have been achieved and how the individual/team/project/ initiative has made a difference.

Before completing the form think about what makes your team/project a winner - show imagination, vision and enthusiasm in your submission. Your entry needs to engage the judges as they will be reading many other submissions.

Giving examples will assist your application - for example, if the project / individual reduced crime, please provide relevant crime statistics / feedback. Relevant supporting material could also include details of budgets and efficiency savings as applicable.

You may also supply photographs or other supporting material of the individual or the work undertaken in the project as supporting information.

N.B: Projects must have been completed (or mostly completed) between 1 April 2009 and 26 February 2010
